Helen R. Cook/Campbell ( Maiden name Helen Nicklay) 74 of Middletown NJ died Wednesday, May 17, 2006 at Riverview Medical Center Red Bank NJ. She was a Registered Nurse for Bayshore Community Hospital, Holmdel, NJ for 10 years, retired 199 3. Prior she worked for Monmouth Medical Center, Long Branch, NJ for 24 years.  She lived in Middletown NJ for many years. She was born in St. Cloud, MN and lived in Perham MN and Barnesville, MN. She was a communicant of St. Catherines RC Church, N. Middletown, and a member of a Bowling League at Harmony Bowl, Middletown.

Surviving are her husband, Donald Cook and daughter, Alice & Gene Tate of Highlands, NJ, son, Michael & Cynthia Campbell of Farmingdale, NJ, daughter, Michele & Don Welch of Jackson, NJ, son, Harry & Melissa Campbell III of Baltimore, MD, sister, Gladys Gossman of Belair, MD, brother, Alan Steinke, of TX, sister, Marge Nicklay of Torrance, CA, 13 grandchildren Ceton, Ryan, Joshua, Grace, Bethany, Julia, Sarah, Emily, Jacob, Madeleine, Ian, Evan and Rowan.  

She was preceded in death by her first husband Harry Campbell Jr, in1970, her mother Lillian Steinke, father Lawrence Nicklay, brother William Nicklay and an infant brother.
