Perham Memorial Home Care is pleased to introduce Sue Zaic as the clinical manager for its Detroit Lakes office. Sue received her Bachelors of Science and Public Health degrees from University of Minnesota Moorhead and her Certified Nurse Practitioner license in Family Medicine through the Tri-College Consortium.

In April, Perham Home Care announced its partnership with Emmanuel Community in Detroit Lakes to provide enhanced home health services to that community and has since opened an office there.

As clinical manager, Sue is responsible for the daily administrative operations, including patient admission assessments, client visits, patient, family, and staff education, and the coordination of services and staffing between Perham Memorial Home Care and Emmanuel Community. Emmanuel Community includes Emmanuel Nursing Home, The Madison and Lamplighter Manor Apartments, and the Center for Orthopedic Rehabilitation Excellence.

Sue brings to Perham Home Care immense experience in the home care industry, including the start-up of a hospice agency, three home care agencies, a respite care program, and five assisted living facilities. Prior to joining the Perham agency, Sue was manager of community and outreach services at Minnewaska Home Care.

Sue welcomes inquiries about home care services by contacting her at 218-844-7158 or szaic@pmhh.com .
